Convened physician and management leaders along with governance and community leaders to develop a single clinical services vision and plan for each specialty care service
Each plan included an in-depth analysis of the continuum of care with specific strategies and tactics developed around care coordination, access to care (including the sites of care) and care management. Each plan also included a structure to govern, manage and brand the new delivery model around world-class specialty care.
